Replication not removed from processing

We recently experienced an issue where a replication was submitted but not removed from processing. It appears that a replication was submitted twice for processing, something which shouldn't occur. It then follows that the ACE Token Store was uploaded twice to the ACE AM, and one of those responses resulted in a 500. After this sequence of events the bag was no longer able to be submitted.
